Responsibilities:

• Conduct comprehensive nursing assessments for patients in home health, rehabilitation, or long-term care settings and develop individualized plans of care
• Administer prescribed medications, IV therapies, wound care, and other clinical treatments in accordance with physician orders and facility or agency protocols
• Monitor and document patient vital signs, functional progress, and changes in condition, communicating findings promptly to the interdisciplinary care team
