Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehabilitation Settings Across Akron

Responsibilities:

• Provide direct patient care including medication administration, wound care, and vital sign monitoring in LTC, home health, or rehab settings
• Collaborate with registered nurses, physicians, and interdisciplinary care teams to implement and update individualized patient care plans
• Conduct thorough patient assessments and document clinical observations accurately in electronic health record (EHR) systems
• Assist residents and patients with activities of daily living (ADLs) including bathing, grooming, mobility support, and nutrition
• Monitor patients for changes in condition and communicate critical updates promptly to supervising RNs or attending physicians
• Administer prescribed medications and treatments in accordance with Ohio Nursing Practice Act standards and facility protocols
• Provide education and emotional support to patients and their families regarding care plans, medications, and discharge instructions
• Maintain accurate and timely clinical documentation to meet regulatory, compliance, and accreditation requirements
• Assist with rehabilitative exercises and therapeutic activities under the direction of physical or occupational therapy staff
• Ensure a safe, clean, and dignified care environment in alignment with infection control standards and facility policies

What You'll Need to Qualify as an LPN/LVN in Akron, OH
