Part-Time Licensed Practical & Vocational Nurse Opportunities in Long-Term Care, Home Health & Rehabilitation

Responsibilities:

• Administer prescribed medications and treatments in accordance with New York State nursing practice standards and facility protocols
• Monitor and document patient vital signs, health status changes, and responses to care plans across LTC, home health, or rehab settings
• Assist residents and patients with activities of daily living (ADLs) including bathing, dressing, mobility, and nutritional support
• Perform wound assessments, dressing changes, and basic wound care procedures under the direction of a supervising RN or physician
• Communicate patient condition updates effectively to registered nurses, physicians, and interdisciplinary care team members
• Provide compassionate in-home nursing care to homebound patients in the Rochester area, including post-surgical and chronic disease management support
• Support rehabilitation patients in achieving functional recovery goals by assisting with therapeutic exercises and documenting progress accurately
• Educate patients and their families on medication regimens, discharge instructions, and ongoing home care best practices
• Maintain accurate and timely clinical documentation in electronic health record (EHR) systems in compliance with facility and state requirements
• Uphold infection control standards, safety protocols, and HIPAA compliance in all care environments
