Part-Time Licensed Practical Nurse Opportunities Across Long-Term Care, Home Health & Rehab Settings in the Philadelphia Area
Responsibilities:
- Administer prescribed medications and treatments in accordance with Pennsylvania State Board of Nursing regulations and facility protocols across LTC, home health, and rehab settings
- Monitor and document patient vital signs, changes in health status, and responses to care plans, reporting significant findings to the supervising RN or physician
- Provide direct, hands-on personal care including wound care, catheter care, and assistance with activities of daily living (ADLs) for patients in long-term care and rehabilitation environments
- Conduct home health visits throughout the Philadelphia area, delivering skilled nursing services and educating patients and family caregivers on ongoing care procedures and medication management
- Collaborate with interdisciplinary care teams — including physicians, occupational therapists, social workers, and discharge planners — to support seamless patient transitions and outcomes
- Maintain accurate, timely clinical documentation in electronic health record (EHR) systems in compliance with facility, state, and CMS requirements
- Assist in the development and implementation of individualized patient care plans under the direction of a registered nurse or attending physician
- Perform routine assessments of residents in long-term care facilities, identifying early signs of decline and escalating concerns through appropriate clinical channels
- Support rehabilitation patients with therapeutic exercises, mobility assistance, and encouragement to meet functional recovery goals as directed by the therapy team
- Uphold infection control standards, patient privacy (HIPAA compliance), and safety protocols consistent with Philadelphia-area facility policies and Pennsylvania Department of Health regulations
Qualifications & Licensure Requirements for Philadelphia LPN/LVN Candidates
- Active, unrestricted LPN or LVN license issued by the Pennsylvania State Board of Nursing (or a compact state license with intent to work in PA)
- Graduation from an accredited practical or vocational nursing program recognized by the Pennsylvania State Board of Nursing
- Current CPR/BLS certification through the American Heart Association or American Red Cross
- Experience in one or more of the following settings is a plus but not required: long-term care/skilled nursing, home health, or inpatient/outpatient rehabilitation — candidates at all experience levels are encouraged to apply
- Familiarity with electronic health record (EHR) platforms such as PointClickCare, MatrixCare, or similar systems used in LTC and home health environments
- Strong clinical assessment skills with the ability to identify and escalate changes in patient condition promptly and effectively
- Demonstrated ability to work both independently — as required in home health settings — and collaboratively within a multidisciplinary care team
- Reliable transportation and valid driver's license for candidates pursuing home health assignments in the Philadelphia metropolitan area and surrounding Pennsylvania counties
