Part-Time Licensed Practical Nurse Opportunities Across Long-Term Care, Home Health & Rehab Settings in the Philadelphia Area

Responsibilities:

• Administer prescribed medications and treatments in accordance with Pennsylvania State Board of Nursing regulations and facility protocols across LTC, home health, and rehab settings
• Monitor and document patient vital signs, changes in health status, and responses to care plans, reporting significant findings to the supervising RN or physician
• Provide direct, hands-on personal care including wound care, catheter care, and assistance with activities of daily living (ADLs) for patients in long-term care and rehabilitation environments
• Conduct home health visits throughout the Philadelphia area, delivering skilled nursing services and educating patients and family caregivers on ongoing care procedures and medication management
• Collaborate with interdisciplinary care teams — including physicians, occupational therapists, social workers, and discharge planners — to support seamless patient transitions and outcomes
• Maintain accurate, timely clinical documentation in electronic health record (EHR) systems in compliance with facility, state, and CMS requirements
• Assist in the development and implementation of individualized patient care plans under the direction of a registered nurse or attending physician
• Perform routine assessments of residents in long-term care facilities, identifying early signs of decline and escalating concerns through appropriate clinical channels
• Support rehabilitation patients with therapeutic exercises, mobility assistance, and encouragement to meet functional recovery goals as directed by the therapy team

Qualifications & Licensure Requirements for Philadelphia LPN/LVN Candidates
