Part-Time LPN/LVN Opportunities Across Long-Term Care, Home Health & Rehab Settings in NYC
Responsibilities:
- Administer medications and treatments as prescribed, accurately documenting all interventions in compliance with New York State Department of Health regulations and facility policies
- Monitor and record vital signs, patient intake and output, and changes in condition, promptly escalating concerns to the supervising RN or physician
- Provide direct patient care including wound care, catheter care, tube feedings, and other skilled nursing procedures within LPN/LVN scope of practice
- Assist in developing and implementing individualized care plans in collaboration with the interdisciplinary team across LTC, home health, or rehabilitation settings
- Deliver compassionate personal care and activities of daily living (ADL) support to residents, home-bound patients, or rehab clients in alignment with their care goals
- Educate patients and family members on medication regimens, discharge instructions, disease management, and home safety in an accessible, culturally sensitive manner
- Maintain accurate, timely, and thorough electronic health records (EHR) documentation to ensure continuity of care and regulatory compliance
- Collaborate with physical therapists, occupational therapists, social workers, and other healthcare team members to support patient recovery and rehabilitation milestones
- Perform admission, transfer, and discharge processes including patient assessments, paperwork coordination, and communication with receiving care teams
- Uphold infection control standards, HIPAA guidelines, and facility-specific protocols to ensure a safe environment for patients, residents, and colleagues
Qualifications We Look For in Our New York LPN/LVN Candidates
- Current, active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license issued by the New York State Education Department (NYSED), in good standing with no restrictions
- Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross, or ability to obtain prior to first assignment
- Experience in one or more of the following settings preferred but not required: long-term care/skilled nursing facility, home health, or inpatient/outpatient rehabilitation
- Demonstrated ability to perform skilled nursing procedures including wound care, medication administration, IV therapy (where permitted by scope), and patient assessments
- Strong written and verbal communication skills, with the ability to document clearly in electronic health record (EHR) systems and communicate effectively with multidisciplinary teams
- Ability to work flexible part-time shifts including days, evenings, nights, or weekends, depending on facility and patient needs across the New York City metro area
- Compassionate, patient-centered approach to care with a demonstrated ability to work with diverse patient populations across New York's multicultural communities
- Reliable transportation or ability to commute to assigned facilities or patient homes within the five boroughs or surrounding New York metro area
