Responsibilities:

• Assist residents and patients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting in LTC and rehab settings
• Monitor and accurately document vital signs including blood pressure, pulse, temperature, and respiration rates, reporting changes to the supervising nurse
• Provide compassionate mobility assistance including transfers, repositioning, and ambulation support in accordance with care plans and therapy goals
• Support rehabilitative nursing goals by encouraging patient participation in prescribed exercises and documenting progress for the clinical team
• Maintain a safe, clean, and dignified environment for residents by performing light housekeeping duties specific to patient rooms and common care areas
• Assist with nutritional support by serving meals, encouraging adequate intake, and documenting food and fluid consumption per facility protocols
• Respond promptly to call lights and patient requests, demonstrating empathy and professionalism in all resident and family interactions
• Observe and report changes in resident physical or emotional condition, including signs of pain, skin breakdown, or behavioral changes, to the charge nurse
• Follow all infection control, HIPAA, and facility-specific safety protocols to ensure a compliant and secure care environment
