Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehab Across Music City

Responsibilities:

• Administer prescribed medications and treatments to residents or patients in accordance with Tennessee Board of Nursing standards and facility protocols
• Monitor and document patient vital signs, health status changes, and responses to care interventions in electronic health record (EHR) systems
• Provide direct bedside nursing care in long-term care, home health, or rehabilitation settings under the supervision of a Registered Nurse or physician
• Develop and contribute to individualized patient care plans in collaboration with interdisciplinary care teams
• Perform wound assessments, dressing changes, catheter care, and other clinical procedures as delegated and within LPN/LVN scope of practice
• Communicate effectively with patients, family members, and healthcare team members to ensure continuity and quality of care
• Assist with patient admissions, discharges, and transitions of care including thorough documentation and handoff reporting
• Support rehabilitation patients with therapeutic exercises, mobility assistance, and ADL (activities of daily living) as directed by therapy teams
• Conduct home health visits across Nashville and surrounding Middle Tennessee communities, ensuring safe and patient-centered care in residential settings
• Maintain compliance with HIPAA regulations, infection control standards, and all applicable state and federal healthcare guidelines

Qualifications We're Looking For in Nashville LPN/LVN Candidates

• Active and unrestricted LPN or LVN license issued by the Tennessee Board of Nursing (or a Nurse Licensure Compact license in good standing)
• Graduation from an accredited practical or vocational nursing program recognized by the Tennessee Board of Nursing
• Current CPR/BLS certification from the American Heart Association or American Red Cross
• Experience in long-term care, home health, or rehabilitation nursing is preferred but not required — new graduates are encouraged to apply
• Proficiency with electronic health record systems; experience with PointClickCare, MatrixCare, or similar LTC platforms is a plus
• Strong clinical assessment skills and the ability to recognize and communicate changes in patient condition promptly
• Reliable transportation and valid Tennessee driver's license for home health assignments requiring travel within the Nashville metro area
• Ability to pass a comprehensive background check, drug screening, and health clearance as required by partnering healthcare facilities
