Part-Time Licensed Practical Nurse Opportunities Across Long-Term Care, Home Health & Rehabilitation Settings

Responsibilities:

  • Provide direct patient care in long-term care, home health, or rehabilitation settings under the supervision of a Registered Nurse or physician
  • Monitor and document patient vital signs, health status changes, and responses to treatment in accordance with facility protocols
  • Administer prescribed medications and treatments accurately, following North Carolina nursing scope of practice guidelines
  • Assist residents and patients with activities of daily living (ADLs) including personal hygiene, mobility support, and nutrition
  • Collaborate with interdisciplinary care teams — including RNs, therapists, and social workers — to develop and implement individualized care plans
  • Perform wound assessments and dressing changes, maintaining proper infection control standards at all times
  • Communicate effectively with patients, families, and caregivers to provide education and ensure understanding of care plans and discharge instructions
  • Conduct home health visits across the Winston-Salem area, assessing patient environments for safety and documenting observations thoroughly
  • Support rehabilitation patients with therapeutic exercises and recovery protocols as directed by the supervising clinical team
  • Maintain accurate and timely electronic health records (EHR) documentation in compliance with facility, state, and federal regulations

Qualifications & Requirements for LPN/LVN Candidates in Winston-Salem

  • Active and unrestricted LPN or LVN license issued by the North Carolina Board of Nursing (NCBON)
  • Graduation from an accredited Licensed Practical Nursing or Licensed Vocational Nursing program
  • Current CPR/BLS certification through the American Heart Association or American Red Cross
