Part-Time Licensed Practical & Vocational Nurse Opportunities in Long-Term Care, Home Health & Rehabilitation
Responsibilities:
- Administer prescribed medications and treatments in accordance with North Carolina Board of Nursing standards and facility or agency protocols
- Monitor and document patient vital signs, health status changes, and responses to treatment plans in long-term care, home health, or rehabilitation settings
- Collaborate with registered nurses, physicians, and interdisciplinary care teams to implement and update individualized patient care plans
- Provide direct patient care including wound care, catheter management, ostomy care, and assistance with activities of daily living (ADLs)
- Conduct initial and ongoing assessments of patient conditions and communicate significant changes to supervising RNs or attending physicians promptly
- Educate patients and their families or caregivers on medication regimens, disease management, post-acute recovery expectations, and safe home care practices
- Maintain accurate and timely clinical documentation using electronic health record (EHR) systems in compliance with facility, agency, and regulatory standards
- Assist with rehabilitation exercises and mobility support under the direction of physical and occupational therapists in rehab care environments
- Ensure infection control protocols, safety standards, and HIPAA compliance are upheld in all patient care interactions and environments
- Participate in care conferences, shift handoffs, and quality improvement initiatives to support optimal patient outcomes and team communication
Qualifications & Licensing Requirements for LPN/LVN Candidates in North Carolina
- Active and unrestricted LPN or LVN license issued by the North Carolina Board of Nursing (NCBON) or a valid Nurse Licensure Compact (NLC) license
- Graduation from an accredited practical or vocational nursing program recognized by the NCBON
- Current Basic Life Support (BLS) certification through the American Heart Association or American Red Cross
- Experience in long-term care, home health, skilled nursing, or rehabilitation settings is preferred but candidates of all experience levels are encouraged to apply
- Strong knowledge of medication administration procedures, documentation requirements, and infection control standards
- Ability to work independently and exercise sound clinical judgment, particularly in home health environments with minimal direct supervision
- Reliable transportation and the ability to travel to patient homes or multiple facility locations within the Raleigh, NC metro area as required
- Proficiency or willingness to train on electronic health record (EHR) platforms commonly used in long-term care and home health settings
