Responsibilities:
- Assist patients and residents with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting across LTC, rehab, and home health settings
- Monitor and document vital signs such as blood pressure, pulse, temperature, and respiration rates, reporting any changes to the supervising nurse promptly
- Support safe patient mobility through transfers, repositioning, and ambulation assistance using proper body mechanics and assistive devices
- Provide compassionate companionship and emotional support to patients and residents, fostering dignity and respect in every interaction
- Assist with nutritional needs including meal preparation support, feeding assistance, and monitoring of food and fluid intake
- Maintain a clean, safe, and comfortable environment for patients by performing light housekeeping duties and ensuring infection control protocols are followed
- Communicate effectively with interdisciplinary care teams, including nurses, therapists, and social workers, to support coordinated patient care plans
- Observe and report changes in patient behavior, condition, skin integrity, or mood that may indicate medical or emotional concerns
- Assist rehabilitation patients with prescribed therapeutic exercises under the direction of licensed therapists and nursing staff
- Accurately complete required documentation including care logs, incident reports, and electronic health record (EHR) entries in compliance with facility and state standards
Qualifications We Look For in Our Wilmington CNA Candidates
- Active and unrestricted Certified Nursing Assistant (CNA) certification in the state of Delaware through the Delaware Health Care Facility Licensing and Certification program
- Current listing in good standing on the Delaware Nurse Aide Registry with no substantiated findings of abuse, neglect, or misappropriation
- Minimum completion of a state-approved CNA training program; prior clinical experience in LTC, rehabilitation, or home health preferred but not required
- Valid CPR/BLS certification from an accredited provider such as the American Heart Association or American Red Cross
- Demonstrated ability to perform physical demands of the role including lifting, transferring, and standing for extended periods during a shift
- Strong interpersonal and communication skills with a patient-centered approach to care delivery across diverse populations
- Ability to pass a comprehensive background check, drug screening, and health clearance including TB testing as required by Delaware healthcare regulations
- Reliable transportation and flexibility to work across various Wilmington-area facilities or client homes depending on assignment type
