Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehab Settings Across Aurora, CO
Responsibilities:
- Provide direct patient care including medication administration, wound care, and vital sign monitoring in LTC, home health, or rehab settings
- Collaborate with RNs, physicians, and interdisciplinary care teams to implement and update individualized patient care plans
- Document patient assessments, interventions, and progress accurately within electronic health record (EHR) systems in compliance with Colorado state regulations
- Assist with Activities of Daily Living (ADLs) and support patient mobility, hygiene, and nutrition needs, particularly in long-term care environments
- Conduct skilled nursing visits in home health settings, educating patients and family members on medication management, disease processes, and recovery protocols
- Monitor patients for changes in condition and report critical findings promptly to supervising RNs or attending physicians
- Support rehabilitation patients with therapeutic exercises, ambulation assistance, and post-procedure recovery care under the direction of physical and occupational therapists
- Maintain infection control standards and adhere to facility-specific and OSHA safety protocols to protect patients and staff
- Participate in care conferences, shift handoffs, and interdisciplinary team meetings to ensure continuity and quality of patient care
- Maintain accurate supply inventory and ensure proper handling and documentation of controlled substances per Colorado Board of Nursing guidelines
Qualifications & Licensing Requirements for LPN/LVN Candidates in Colorado
- Active and unrestricted LPN or LVN license issued by the Colorado State Board of Nursing (or a Nurse Licensure Compact license recognized in Colorado)
- Graduate of an accredited practical or vocational nursing program approved by a recognized accrediting body
- Current CPR/BLS certification from the American Heart Association or American Red Cross
- Minimum of 0–1 year of clinical experience preferred; new graduates with strong clinical rotations in LTC, home health, or rehab settings are encouraged to apply
- Familiarity with electronic health record platforms such as PointClickCare, MatrixCare, or similar systems used in post-acute and home health environments
- Ability to pass a comprehensive background check, drug screening, and reference verification as required by Colorado state law and facility credentialing standards
- Strong communication, time management, and critical thinking skills suited to fast-paced or autonomous healthcare environments
- Reliable transportation and the ability to travel to patient homes or multiple facility locations within the Aurora and greater Denver metro area, as applicable to home health assignments
