Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehab Settings Across Aurora, CO

Responsibilities:

• Provide direct patient care including medication administration, wound care, and vital sign monitoring in LTC, home health, or rehab settings
• Collaborate with RNs, physicians, and interdisciplinary care teams to implement and update individualized patient care plans
• Document patient assessments, interventions, and progress accurately within electronic health record (EHR) systems in compliance with Colorado state regulations
• Assist with Activities of Daily Living (ADLs) and support patient mobility, hygiene, and nutrition needs, particularly in long-term care environments
• Conduct skilled nursing visits in home health settings, educating patients and family members on medication management, disease processes, and recovery protocols
• Monitor patients for changes in condition and report critical findings promptly to supervising RNs or attending physicians
• Support rehabilitation patients with therapeutic exercises, ambulation assistance, and post-procedure recovery care under the direction of physical and occupational therapists
• Maintain infection control standards and adhere to facility-specific and OSHA safety protocols to protect patients and staff
• Participate in care conferences, shift handoffs, and interdisciplinary team meetings to ensure continuity and quality of patient care
• Maintain accurate supply inventory and ensure proper handling and documentation of controlled substances per Colorado Board of Nursing guidelines

Qualifications & Licensing Requirements for LPN/LVN Candidates in Colorado

• Active and unrestricted LPN or LVN license issued by the Colorado State Board of Nursing (or a Nurse Licensure Compact license recognized in Colorado)
• Graduate of an accredited practical or vocational nursing program approved by a recognized accrediting body
• Current CPR/BLS certification from the American Heart Association or American Red Cross
• Minimum of 0–1 year of clinical experience preferred; new graduates with strong clinical rotations in LTC, home health, or rehab settings are encouraged to apply
