Part-Time Licensed Practical & Vocational Nurse Opportunities in LTC, Home Health & Rehabilitation Settings Across the Denver Metro Area
Responsibilities:
- Administer prescribed medications and treatments in accordance with Colorado Board of Nursing standards and facility protocols across LTC, home health, and rehab settings
- Monitor and document patient vital signs, health status changes, and responses to care plans, escalating concerns promptly to supervising RNs or physicians
- Assist in developing, implementing, and updating individualized patient care plans in collaboration with interdisciplinary care teams
- Provide skilled wound care, catheter management, IV therapy (where permitted under CO scope of practice), and other clinical procedures as assigned
- Deliver direct patient care including personal hygiene assistance, mobility support, and comfort measures for long-term care and home health patients
- Educate patients and family members on medication regimens, chronic disease management, and post-discharge or home care instructions
- Collaborate with physical, occupational, and speech therapists in rehabilitation environments to reinforce therapeutic goals during nursing interactions
- Maintain accurate and timely electronic health record (EHR) documentation in compliance with facility, state, and federal regulatory requirements
- Conduct thorough head-to-toe patient assessments at the start of each shift and communicate relevant findings to charge nurses and care coordinators
- Uphold patient dignity, confidentiality, and rights in accordance with HIPAA regulations and facility-specific policies across all care settings
Qualifications & Licensing Requirements for Denver LPN/LVN Candidates
- Active and unrestricted LPN or LVN license issued by the Colorado Board of Nursing (or a Nurse Licensure Compact license recognized in Colorado)
- Graduation from an accredited practical or vocational nursing program approved by the state of Colorado or the candidate's home compact state
- Current CPR/BLS certification from the American Heart Association or American Red Cross
- Ability to meet pre-employment requirements including background check, drug screening, and health clearance per facility and agency standards
- Demonstrated ability to work independently and prioritize care needs in home health environments or as part of a team in LTC and rehab settings
- Basic proficiency with electronic health record (EHR) systems; experience with PointClickCare, MatrixCare, or similar platforms is a plus
- Reliable transportation and valid driver's license required for home health assignments within the greater Denver metropolitan area
