Part-Time LPN & LVN Opportunities in Long-Term Care, Home Health & Rehabilitation

Responsibilities:

• Administer prescribed medications and treatments in accordance with Colorado Nurse Practice Act standards and facility protocols
• Monitor and document patient vital signs, health status changes, and responses to treatment in long-term care, home health, or rehabilitation environments
• Assist with the development and implementation of individualized patient care plans under the supervision of a Registered Nurse or physician
• Provide wound care, catheter care, tube feedings, and other skilled nursing interventions as ordered
• Communicate effectively with interdisciplinary care teams, patients, and family members to ensure continuity and quality of care
• Conduct thorough patient assessments and report significant changes in condition to the supervising RN or attending provider promptly
• Support activities of daily living and assist patients with mobility, hygiene, and rehabilitative exercises as directed by therapy staff
• Maintain accurate and timely clinical documentation in electronic health record (EHR) systems consistent with facility and regulatory requirements
• Educate patients and caregivers on disease management, medication adherence, and discharge or home care instructions
• Adhere to infection control protocols, HIPAA regulations, and all applicable state and federal healthcare compliance standards

Qualifications & Licensing Requirements for LPN/LVN Candidates in Colorado

• Active and unrestricted LPN or LVN license issued by the Colorado State Board of Nursing (or compact state licensure eligible for Colorado practice)
• Graduation from an accredited practical or vocational nursing program
• Current CPR/BLS certification from the American Heart Association or American Red Cross
• Minimum of one year of clinical experience preferred; new graduates with clinical rotations in LTC, home health, or rehab settings are encouraged to apply
• Demonstrated ability to administer medications, perform skilled nursing procedures, and document care accurately
• Familiarity with electronic health record (EHR) platforms such as PointClickCare, MatrixCare, or similar systems is a plus
• Strong interpersonal communication skills and a patient-centered approach to care delivery
• Ability to pass a pre-employment background check, drug screening, and health screening as required by facility partners
