Part-Time Licensed Practical & Vocational Nurse Opportunities in Long-Term Care, Home Health & Rehab

Responsibilities:

• Administer prescribed medications and treatments in accordance with physician orders and Florida State Board of Nursing standards
• Monitor and document patient vital signs, health status changes, and responses to care interventions across LTC, home health, and rehab settings
• Perform wound assessments and dressing changes, including management of pressure ulcers and post-surgical sites in long-term care and rehabilitation environments
• Collaborate with RNs, physicians, and interdisciplinary care teams to develop and update individualized patient care plans
• Provide culturally sensitive, patient-centered care to a diverse Hialeah clientele, including bilingual communication support when applicable
• Assist with activities of daily living (ADLs) and support patient mobility and rehabilitation exercises under the direction of licensed therapists
• Conduct admission, discharge, and routine nursing assessments and accurately maintain electronic health records (EHR)
• Educate patients and family members on medication regimens, disease management, and home health safety protocols
• Communicate critical changes in patient condition promptly to supervising RNs and attending physicians
• Ensure compliance with HIPAA regulations, infection control protocols, and facility or agency-specific policies during all patient interactions

Qualifications & Requirements for LPN/LVN Candidates in Hialeah

• Active and unrestricted LPN or LVN license issued by the Florida Board of Nursing (or a compact state license eligible for Florida practice)
• Current Basic Life Support (BLS/CPR) certification through the American Heart Association or American Red Cross
