Part-Time LPN & LVN Opportunities Across Long-Term Care, Home Health & Rehabilitation Settings in Jacksonville

Responsibilities:

• Administer prescribed medications and treatments to patients in LTC, home health, or rehabilitation settings in accordance with Florida nursing standards and facility protocols
• Monitor and document patient vital signs, health status changes, and responses to treatment, reporting significant changes to supervising RNs or physicians promptly
• Assist in the development, implementation, and evaluation of individualized patient care plans in coordination with the interdisciplinary care team
• Provide wound care, dressing changes, catheter care, and other skilled nursing procedures as directed by the care plan
• Deliver compassionate personal care assistance and support to patients with activities of daily living (ADLs), maintaining dignity and patient-centered focus
• Educate patients and family members on care procedures, medication management, and discharge or home care instructions
• Maintain accurate, timely, and legally compliant clinical documentation using electronic health record (EHR) systems
• Collaborate with physical therapists, occupational therapists, and other rehabilitation team members to support patient recovery goals in rehab settings
• Observe and report signs of patient distress, infection, or deterioration and initiate appropriate nursing interventions within scope of practice
• Ensure infection control standards and OSHA guidelines are consistently followed across all patient care environments

Qualifications We're Looking For in Jacksonville LPN/LVN Candidates

• Active and unrestricted LPN or LVN license issued by the Florida Board of Nursing (or a Nurse Licensure Compact state with intent to obtain FL licensure)
• Current CPR/BLS certification from the American Heart Association or American Red Cross
• Experience in at least one of the following settings is preferred but not required: long-term care, home health, or inpatient/outpatient rehabilitation
• Solid understanding of medication administration, infection control practices, and documentation standards in a regulated healthcare environment
• Ability to work flexible part-time shifts including days, evenings, or weekends based on facility or patient scheduling needs
• Strong interpersonal and communication skills with the ability to work effectively within interdisciplinary nursing and healthcare teams
• Proficiency or willingness to learn electronic health record (EHR) platforms commonly used in Florida healthcare settings
• Reliable transportation and the ability to travel within the Jacksonville metro area for home health or multi-site assignments as applicable
