Part-Time Licensed Practical & Vocational Nursing Opportunities in LTC, Home Health & Rehab Settings Across Greater Houston
Responsibilities:
- Administer prescribed medications and treatments in accordance with physician orders and Texas Board of Nursing standards
- Monitor and document patient vital signs, health status changes, and responses to care in long-term care, home health, or rehabilitation settings
- Provide skilled wound care, catheter care, and other clinical procedures as directed by the supervising RN or physician
- Assist residents and patients with activities of daily living (ADLs) while promoting independence and dignity throughout the care process
- Collaborate with interdisciplinary care teams — including RNs, physical therapists, occupational therapists, and social workers — to implement individualized care plans
- Educate patients and family members on medication regimens, discharge instructions, and chronic disease management strategies
- Conduct thorough patient assessments and communicate changes in condition promptly to the charge nurse or clinical supervisor
- Maintain accurate and timely medical records and nursing documentation in compliance with facility, state, and federal regulations
- Support infection control protocols and uphold standards for a safe, sanitary care environment across all assigned settings
- Provide compassionate end-of-life or palliative support to residents and their families in long-term care environments when applicable
Qualifications & Requirements for Houston LPN/LVN Candidates
- Active and unrestricted LPN or LVN license issued by the Texas Board of Nursing (or compact state license with Texas privileges)
- Current CPR/BLS certification from the American Heart Association or American Red Cross
- Minimum of one (1) year of clinical experience preferred; new graduates with clinical rotations in LTC, home health, or rehab settings are encouraged to apply
- Demonstrated knowledge of nursing best practices for geriatric, post-acute, or rehabilitation patient populations
- Proficiency in electronic health record (EHR) documentation systems; experience with PointClickCare, MatrixCare, or similar platforms is a plus
- Strong interpersonal communication skills with the ability to work effectively across diverse patient populations in the Greater Houston area
- Ability to pass a pre-employment background check, drug screen, and applicable health clearances per facility and state requirements
- Reliable transportation and valid driver's license required for home health assignments within Harris County and surrounding communities
