Flexible Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehabilitation Settings Across Austin

Responsibilities:

• Provide direct patient care including medication administration, wound care, and vital sign monitoring in LTC, home health, and rehabilitation settings
• Perform comprehensive nursing assessments and document findings accurately in electronic health record (EHR) systems in accordance with Texas Board of Nursing standards
• Collaborate with registered nurses, physicians, and interdisciplinary care teams to develop and implement individualized patient care plans
• Assist patients with activities of daily living (ADLs) including bathing, dressing, feeding, and mobility support, maintaining dignity and comfort at all times
• Monitor patients for changes in condition and promptly communicate concerns to supervising RNs or attending physicians
• Administer prescribed medications and treatments per physician orders, ensuring accurate documentation and strict adherence to safety protocols
• Educate patients and family members on care plans, discharge instructions, medication management, and chronic disease self-management techniques
• Maintain accurate and timely clinical documentation in compliance with facility policies, CMS regulations, and Texas state licensure requirements
• Support rehabilitation patients by reinforcing therapy goals, assisting with exercises, and motivating progress toward functional independence
• Coordinate with home health aides and care coordinators in community-based settings to ensure continuity and quality of patient care

Qualifications & Licensing Requirements for Austin, TX LPN/LVN Candidates

• Current, active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license issued by the Texas Board of Nursing (or a Nurse Licensure Compact license recognized in Texas)
• Graduation from an accredited LPN/LVN nursing program approved by the Texas Board of Nursing
• Current CPR/BLS certification through the American Heart Association or American Red Cross
• Experience in long-term care, home health, or rehabilitation settings is preferred but not required — all experience levels are welcome to apply
• Proficiency in or demonstrated ability to learn electronic health record (EHR) systems commonly used in post-acute and community care environments
• Strong interpersonal and communication skills with the ability to engage compassionately with elderly, post-surgical, and medically complex patient populations
