Part-Time Licensed Practical & Vocational Nursing Opportunities Across Dallas — LTC, Home Health & Rehab
Responsibilities:
- Administer prescribed medications and treatments in accordance with Texas Board of Nursing (BON) scope of practice guidelines and facility protocols
- Monitor and document patient vital signs, health status changes, and care plan progress across LTC, home health, and rehabilitation settings
- Provide direct wound care, catheter care, tube feeding management, and other skilled nursing interventions as ordered by the supervising physician or RN
- Collaborate with registered nurses, physical therapists, occupational therapists, and interdisciplinary care teams to ensure continuity of patient-centered care
- Conduct patient and family education on medication regimens, discharge instructions, and chronic disease management strategies
- Perform thorough nursing assessments and communicate significant changes in patient condition to supervising RN or physician in a timely manner
- Maintain accurate and compliant clinical documentation in electronic health record (EHR) systems in accordance with facility and regulatory standards
- Assist with ADL (activities of daily living) support and rehabilitation exercises to promote patient independence and functional recovery
- Follow all infection control, HIPAA, and safety protocols to maintain a safe care environment for patients, families, and colleagues
- Participate in care conferences, shift handoffs, and quality improvement initiatives as required by the employing facility or home health agency
Qualifications & Requirements for Dallas LPN/LVN Candidates
- Active, unencumbered LPN or LVN license issued by the Texas Board of Nursing (BON) — or a compact nursing license with Texas as the primary state
- Graduation from an accredited practical or vocational nursing program recognized by the Texas BON
- Current BLS/CPR certification from the American Heart Association or American Red Cross (required prior to placement)
- Ability to meet the Texas Health and Safety Code requirements for background checks and health screenings, including TB testing
- Demonstrated ability to work independently in home health environments and as part of a collaborative team in LTC and rehab settings
- Basic proficiency with electronic health record (EHR) systems; experience with PointClickCare, MatrixCare, or similar platforms is a plus
- Reliable transportation and a valid Texas driver's license for home health assignments within the Dallas-Fort Worth service area
- Strong clinical communication skills and a compassionate, patient-first approach to care delivery across all healthcare settings
