Part-Time Nursing Opportunities in Long-Term Care, Home Health & Rehab Settings Across the Fort Worth Area
Responsibilities:
- Administer medications and treatments in accordance with physician orders and Texas Board of Nursing LVN scope of practice guidelines
- Monitor and document patient vital signs, health status changes, and progress notes in compliance with facility or home health agency protocols
- Provide direct patient care including wound care, catheter care, ostomy management, and IV therapy as permitted under supervision
- Collaborate with registered nurses, therapists, and interdisciplinary care teams to implement and update individualized patient care plans
- Conduct patient and family education on medication regimens, disease management, and post-discharge or home care instructions
- Assist rehab patients with therapeutic exercises and activities of daily living (ADLs) in coordination with physical and occupational therapy staff
- Perform admission assessments, care plan updates, and discharge documentation in long-term care and home health settings
- Maintain accurate and timely electronic health records (EHR) and ensure documentation meets state, federal, and facility compliance standards
- Respond promptly to patient and resident concerns, reporting significant clinical changes to supervising RNs or attending physicians
- Uphold infection control standards, HIPAA regulations, and all applicable Texas Department of Health and Human Services requirements
Qualifications & Licensure Requirements for Fort Worth LPN/LVN Candidates
- Current, active LPN or LVN license issued by the Texas Board of Nursing (or a compact state license with Texas practice eligibility)
- Graduation from an accredited practical or vocational nursing program recognized by the Texas Board of Nursing
- Current CPR/BLS certification from the American Heart Association or American Red Cross
- Experience in one or more of the following settings preferred but not required: long-term care, skilled nursing, home health, or inpatient rehabilitation
- Proficiency with electronic health record (EHR) systems such as MatrixCare, PointClickCare, or similar platforms used in post-acute care environments
- Strong clinical assessment skills and the ability to recognize and escalate changes in patient condition appropriately
- Ability to pass a comprehensive background check, drug screening, and health clearance as required by facility and state regulations
- Reliable transportation and a valid driver's license for home health assignments requiring travel within the Fort Worth metro service area
