Part-Time LPN/LVN Opportunities in Long-Term Care, Home Health & Rehab Across the Alamo City
Responsibilities:
- Administer prescribed medications and treatments in accordance with Texas Board of Nursing scope of practice guidelines for LPN/LVNs
- Monitor and document patient vital signs, health status changes, and progress notes within electronic health record (EHR) systems used by LTC, home health, or rehab facilities
- Provide direct bedside care including wound dressing changes, catheter care, tube feedings, and personal hygiene assistance for patients in long-term care settings
- Conduct in-home patient assessments, coordinate care plans with supervising RNs, and educate patients and family members on medications, disease management, and safety practices
- Support rehabilitation efforts by assisting patients with therapeutic exercises, mobility transfers, and activities of daily living under the direction of physical and occupational therapists
- Communicate effectively with interdisciplinary care teams, physicians, social workers, and family members to ensure continuity of care across all assigned settings
- Observe patients for adverse reactions, complications, or changes in condition and report findings promptly to supervising registered nurses or attending physicians
- Maintain accurate and timely clinical documentation in compliance with facility policies, CMS standards, and Texas state regulatory requirements
- Assist in developing and updating individualized care plans for long-term care residents, home health patients, and rehab clients in collaboration with the RN care team
- Uphold patient dignity, cultural sensitivity, and confidentiality in every interaction, particularly when serving San Antonio's diverse, bilingual patient population
Qualifications We're Looking For in San Antonio LPN/LVN Candidates
- Active and unrestricted LPN or LVN license issued by the Texas Board of Nursing (or a compact state license with Texas practice privileges)
- Graduation from an accredited practical or vocational nursing program recognized by the Texas Board of Nursing
- Current BLS/CPR certification from the American Heart Association or American Red Cross
- Experience in one or more of the following settings is preferred but not required: long-term care/skilled nursing, home health, or inpatient/outpatient rehabilitation
- Demonstrated ability to administer medications, perform clinical procedures, and document care accurately within established scope of practice
- Strong interpersonal and communication skills, with bilingual English/Spanish proficiency considered a significant asset in the San Antonio market
- Ability to work flexible part-time shifts including days, evenings, or weekends based on facility and patient needs
- Reliable transportation and a valid Texas driver's license are required for home health assignments within the San Antonio metro area
