Part-Time Licensed Practical & Vocational Nurse Opportunities in Long-Term Care, Home Health & Rehab

Responsibilities:

• Provide direct patient care including wound care, catheter care, and medication administration under the supervision of an RN or physician in LTC, home health, or rehab settings
• Monitor and document patient vital signs, health status changes, and responses to treatments, reporting significant findings to the supervising registered nurse or provider
• Administer prescribed medications and treatments in accordance with Washington State nursing practice standards and facility or agency policies
• Assist in the development and implementation of individualized care plans in collaboration with interdisciplinary care teams across long-term care and rehabilitation environments
• Conduct initial and ongoing patient assessments to identify needs, changes in condition, and progress toward rehabilitation or home health care goals
• Educate patients and family members on care techniques, medication management, disease processes, and discharge or home care instructions
• Maintain accurate and thorough clinical documentation in electronic health record (EHR) systems in compliance with facility, agency, and regulatory requirements
• Perform skilled nursing procedures such as IV therapy (where permitted by WA State scope of practice), tube feedings, ostomy care, and post-surgical wound management
• Coordinate with home health aides, physical therapists, occupational therapists, and social workers to ensure continuity and quality of patient care
• Adhere to all infection control protocols, HIPAA regulations, and facility or home health agency safety policies to maintain a safe care environment

Qualifications & Requirements for LPN/LVN Candidates in Tacoma

• Current, active Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license issued by the Washington State Department of Health — no restrictions
• Graduation from an accredited practical or vocational nursing program recognized by the Washington State Nursing Care Quality Assurance Commission (NCQAC)
• Current BLS/CPR certification from the American Heart Association or American Red Cross (required prior to placement)
• Experience in one or more of the following settings is preferred but not required: long-term care/skilled nursing, home health, or rehabilitation — candidates at all experience levels are encouraged to apply
