Temporary RN Opportunities in Home Health, Rehabilitation & Long-Term Care Across the Baltimore Area

Responsibilities:

• Conduct comprehensive nursing assessments and develop individualized care plans for patients in home health, rehabilitation, or long-term care settings
• Administer medications, treatments, and therapeutic interventions in accordance with physician orders and Maryland Board of Nursing standards
• Monitor and document patient progress, vital signs, and clinical changes using electronic health record (EHR) systems
• Coordinate care with interdisciplinary teams including physical therapists, social workers, physicians, and home health aides to ensure continuity of care
• Educate patients and family members on disease management, medication adherence, wound care, and post-acute recovery protocols
• Perform skilled nursing procedures such as wound care, IV therapy, catheter management, and ostomy care as appropriate to the care setting
• Identify and promptly report changes in patient condition, safety concerns, or clinical deterioration to the supervising physician or care team
• Maintain accurate, timely, and compliant clinical documentation that meets agency, facility, and Maryland state regulatory requirements
• Support patient transitions between care settings, including discharge planning and communication with receiving providers or family caregivers
• Uphold patient dignity, privacy, and rights in alignment with HIPAA regulations and person-centered care philosophies

Qualifications & Requirements for Baltimore RN Candidates

• Active, unrestricted Registered Nurse (RN) license issued by the Maryland Board of Nursing or a valid Compact Nursing License recognized in Maryland
• Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program
• Current BLS/CPR certification through the American Heart Association or American Red Cross
• Experience in home health, rehabilitation, long-term care, or a related post-acute setting is preferred but not required for all roles
• Proficiency with electronic health records (EHR) platforms; experience with PointClickCare, Kinnser, or similar systems is a plus
• Strong clinical assessment, critical thinking, and patient communication skills
• Ability to work independently in home or community settings with minimal direct supervision when applicable
• Valid driver's license and reliable transportation for home health assignments within the Baltimore metro service area
